When Not To Use This Form

Please do not use this form if one of the following situations apply:

  2. You have an emergency or life-threatening situation. In this case, call 911 or go directly to the nearest hospital emergency center.
  3. You feel that your needs are more urgent, and would like to request an immediate appointment. In this case, call us directly at 866-499-1588 and we will do our best to get you scheduled right away.
