This members-only section of our website is exclusively for your use and we've worked hard to ensure that we've included comprehensive, yet easy-to-understand and fill out, referral forms and other support information that will make your jobs much, much easier.

To access all the pages in this members-only section, you'll need to first register free on our website. Once you've completed the registration process (it will take less than five minutes) you can log-in and begin using all of the great resources we've provided below.

And it's never been easier, quicker, or more stress-free to submit patient referrals, prescriptions, and CMNs using our online referral generator forms, which can be completed online or printed out and filled in manually. Below are links to our four referral forms:

• Consultation Referral Form - Use this form when you want to refer patients to us for consultations with one of our center's board-certified sleep physicians.
  
• Sleep Study Referral Form – Use this form when you wish to refer patients to us for sleep studies - PSGs, MWTs, MSLTs, and CPAP / BiPAP titrations. This form also has an area where you can also request consultations with one of our board-certified sleep physicians.
• CPAP and BiPAP Equipment & Supplies Prescription / Certificate of Medical Necessity (CMN) Form – Use this form for prescriptions for new machines, old machine upgrades, masks, supplies and more.
• Oxygen Equipment & Supplies Prescription / Certificate of Medical Necessity Form - Use this form to prescribe oxygen equipment or supplies.

Just click on the link provided above to access the appropriate form. All of our forms can be downloaded to your computer, printed, and filled out by hand OR you can elect to fill it out online and then print.

Once completed, fax the appropriate form to us toll-free at: 866-499-1288 and we'll take care of the rest!

After we receive your referrals we will follow up directly with your patients to:

• Confirm their insurance benefits
• Schedule appointments, as necessary
• Obtain detailed sleep histories
• Answer any / all questions they may have about our practice or procedures
• Ensure that they have all of our contact information and know where and how to obtain more help if needed
