We've worked hard to ensure that our referral process is easy, quick, and stress free. Below are links to our standard referral / prescription forms. Each can be printed and filled in by hand OR the requested information can be filled out online, printed, and faxed to us.
Once completed, fax the appropriate form to us at: 866-499-1288, and we'll take care of the rest! In other words, after we receive your referrals we will follow up directly with your patients to:
- Confirm your patients' insurance benefits.
- Schedule appointments, as necessary.
- Obtain detailed sleep histories.
- Answer any / all questions they may have about our practice or procedures.
- Ensure that they have all of our contact information and know where and how to obtain more help if needed.

Understanding Our Diagnostic Sleep Reports
Within 48-72 hours of your patients' sleep studies, one of our board-certified physicians will provide you with easy-to-read and understand reports.
Although we gather volumes of information during each sleep study, we condense it down into a manageable – yet comprehensive – diagnostic report.
Part 2 of our report provides specific statistical information gathered during the study, such as:
- Sleep Data (total recording time, sleep efficiency, sleep period time, etc).
- Sleep Data (min and percentage in each stage of sleep).
- Arousal Summary.
- Respiratory Data.
- Body Position Data.
- Snoring Data.
- Oximetry Distribution.
- Heart Rate Summary.
- Heart Rate Observations.
It is important to note that reports will vary based on the type of study. For instance, a CPAP titration report would include a brief summary of the mask used, a titration pressure table, and more in-depth hypnogram.
